Make an Appointment
Patients are seen by appointment. Same day appointments are made, if possible. To schedule an appointment, please call (855) 871-5220 during regular business hours.
We make every effort to stay on schedule although emergencies sometimes arise. If we are seriously delayed, we will attempt to notify patients with as much notice as possible.

New Patients
New patients are asked to arrive at least 30 minutes early to complete and process your paperwork. You can download, print, and complete all necessary forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking including dosage, recent radiology testing, images and reports and your insurance card to your appointment.
If you are a patient that recently had a procedure or surgery performed, please refer to our post op and discharge instructions. If you have any questions after reviewing, please call our office at (855) 871-5220 to answer any additional questions.
Billing
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.
Insurance
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
Contact your primary care physician to obtain a referral from your insurance carrier, if it is required by your insurance. If this referral is not obtained by the time of your visit, you will be rescheduled.
We recommend that you call your insurance company to verify that we participate with your plan. Here is a list of insurance our practice accepts
Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.
Cancellations or Rescheduling
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

After-Hours & Emergencies
If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (855) 871-5220 and our answering service will assist you.
