If you are unable to find a desired time, or to reschedule an appointment, please contact the office at 972-964-8500 and the receptionist will be happy to accommodate you.

In order to expedite your visit, please bring the following items:

• A list of current medications.

• Insurance card(s) and a Photo ID.

• If you have previously had any X-Rays, CT or MRI testing please bring the images and reports with you to help Dr. Tompkins gain a more thorough understanding of your condition. Due to privacy regulations, if Dr. Tompkins did not order these tests, we cannot request the results be sent to us. 

New Patients, in order to finalize your registration process, please arrive 30 minutes before your scheduled appointment time. 

For questions about insurance and billing, call us directly at (972) 498-4389.

Please note, that if your primary insurance is through Medicaid, our providers are only accepting existing patients. We apologize for the inconvenience, but we cannot see any new Medicaid patients at this time.

As we move into the 2018, please be advised that all demographic and insurance information will need to be updated.  This also applies to all referrals on file for HMO policyholders.  If there are any questions, please call your physician’s office for immediate follow-up.
